Perhaps you customized the wrong file. Maybe there was more than one. Did you check?

i wonder if AutoCAD has a cache somewhere for custom icon buttons?

I have found in the past when making custom icons that autocad will let you browse to the icon you want and when you save it (even if you save as), it will change the name of the BMP to something like "RCDATA16.BMP".
Now what I think happens is, icon names can get doubled up when saved and they default to the name above in more than one folder, so now the custom toolbutton has two of the same named icons to choose from. Somehow, (maybe the order of the folders path'd in autocad) it usually selects the wrong one and we get a different icon than we want on a restart of autocad.
I have custom CUI files I load that contain all of the toolbars I want up, including my custom buttons I have made. I just turn off all autocad tolbars and use my own. In my old original MNS files, and the CUI files I have now, I have the BMP name hardcoded (so to speak) so I know exactly which one it will look for when loading.
Check in the CUI and see what the filenames are for the large and small images used for each toolbutton. If they don't look familiar to the ones you made, that may be your problem.
Hope my rambling makes some sense to you.
