chamfering problem

I am trying to create this type of buttons

when I try to chamfer it. I get this:

How can I fix this?

 

Is there a better way to create this without any script or plugins?

There is a tutor http://www.todddaniele.com/tutorial-Lesson-01.htm but it uses a plugin

 

Any help would be greatly appreciated. thanks.

what did it look like before the chamfer? It looks like some vertexs needed welding before the chamfer. or you need to cut some edges in places plat won't affect your chamfer.

Go into Edge sub object mode, under the Edit Edges menu you heave an option Turn, just activate it, and you will see dashed edges, click on the ones that are above the holes and that should fix it. but you need to connect circles with the outer edge on a similar way as shown on the pic, cos you cant Turn the main edges, just first connect two closest vertices on the circles, and than delete the other edge that was connecting circle to the outher edge. remember allways connect vertices first, and than delete other edge cos it won't work other way arround.
