Very strange never had a problem with sheet set manager until now.

Select files to plot & open no issues i get whats in the sheet set properties on my pdf's , but if i use plot in background...

I have, i guess it also depends on the image your given to insert by the graphics department but many time i have inserted only to find a blank square after saving & re-opening the dwg, i do indeed...

+ there are limits to the quality/size of the image's when using copy paste option.

Your annotating different size MT text that is stacked, in layout1 if you unlock the main (top) viewport & pan up you will see all the MT txt with the issues @ 1:50.

Your annotative text style isn't in a block etc is it ?

Maybe the drawing has been saved with Save to Legacy Drawing File Format...

ps i think your not doing anything wrong its just how you have set things up to display in model space.

Just a guess, but by looking at your icons id say you have autoscale turned on & show objects in all scales on & you have ended up with what i have in the picture, try turning autoscale off & show...
