The behavior happens when I physically print the drawing. Also, I've tried posting here, but it doens't work even when the file size is below 1 MB.

I could e-mail you the drawing, but I'm unable to post drawings here. Maybe it's that I'm at work and a lot of things are blocked.

Are you selecting "Go Advanced" to lower right of reply box? Then select "Manage Attachments".

    Default Here is the file.

I had to WBLOCK this one also to reduce the file size.

So you think it will work now? If so, what did you do? Or are you just putting it up here for others?

I think he put it up for others who might be interested in taking on the challenge of figuring out what is happening.

It plotted fine for me with the dwg to pdf.pc3. I don't have 2012 though so maybe there is some difference there. I also didn't have the xref'd image, but attached my own and everything worked like normal.

Hopefully someone with your version can test it out.

A sample plotted fine for me too. I windowed a portion to get a little bit of everything including structures, numbers, roadway, curved multi-leaders and some of the larger text. My test plot was done on a Canon iP3500 inkjet printer using a sheet size of 8.5x11. I used plain AutoCAD 2010 to open the test drawing.
