Multiple Visibility states and change text header via Lookup

I have 30+ electrical blocks which I am amending, so when you click the Vis. grip you get a list of 30 descriptions and using the Hide/Show options, I have created 1 block that change its contents to suit which description the user chooses..

 

However, The header of the blocks, also needs to be able to be changed, using I think lookup tables.. but Im stuck....

 

I need a list to say "Termination Type 1, Termination Type 2 etc" as a header so no matter which Vis. description you pick, you can amend the header to say "Termination Type 1" etc

 

All help appreciated...
