We don't use the out of the box fixtures but... I just tried it with an MV Part and it worked fine. Are you using the MEP MV Parts?

i'm using tool palettes and when i insert toilet it's too big like 25.4 to big
in viewport drawing is imperial and in modle space its decimal ??

Is the annotation scale in model space set the same as viewport scale in paper space?

That's it . I've also seemed to have lost the lav section in my tool palettes do you know how i can get it back?

Try to right click the icon at the bottom left corner of the tool palette and select customize palettes. You should be able to drag what you want from the left to the right. Just make sure you are putting it on the right palette.
