Title Block Revision help

A co-worker is getting a message that says "Autocad error - Revision attributes are empty" when attempting to add a revision line to his title block in layout space.

 

He does have the proper directory containing the revision drawings, and, supposedly, his computer has the same settings as mine (which works fine).

 

Any idea what is happening?
