dialog vba or lisp help

I would like to start off by stating I am not a programmer, but want to learn how to program auto cad. I am probably starting off to big but hey they say go big or go home. What we want to do is have a dialog box prompt you for answers to questions. As you answer these questions the program would insert predawn blocks for you to finish the layout. Like I said before I am not a programmer but I want to learn so I am not asking someone to do this programming for me. What I would like is if anyone has a example like this that the could let me see use it. So I could see how it works and figure out how to make it work for us. I included the PDF just to show what the questions would look like and how the answers would come. This is adjustable to work with what autoCAD needs.

P.S. i wasn't able to get the example in the top link to work, might be for an older version of acad? however the site is still extremely helpful in general
