LT Dynamic Block w/ Text?

Hi, i'm trying to create a leader with a dynamic block attached, i'm using LT 2008 and i'm not sure how to proceed,

 

i can insert a block to the end of the leader but i haven't yet figured how to make the text dynamic so that the leaders are sequencial, please see attached image,

 

can you help?
