How to create multileader with block and mtext

How can create New Mleader with block and MText also. (like attached file).

Add an attribute, or multiple attributes to the block, that is what I have setup for one of mine.

I simply meant to add an attribute to the block off to the side where your example shows Mtext. You can have a multiple line attribute, while in the attribute definition dialog box add a check mark to the Multiple Lines box on the Left side near the middle. Or add several lines of attributes one below the next.
