-INSERT different block names duplicate definitions

This is an odd problem I am having.  I have two separate, uniquely identified blocks, but when I go to insert it will only fetch one of them and insert it twice.  

 

I get the following error: FLGRNG12 ignored.  

 

This is odd, as each insertion calls a .dwg file generated by WBLOCK - there's no underlying blocks as a possible source of collision.  Not every block has this problem, only certain ones.  

 

Unfortunately these are production blocks so I cannot share them, but I'm hoping someone here will have encountered this before and knows how to fix it.

How are you inserting them? with full path or just the name?

It was with just the name, they were located in a shared folder.  

 

Ultimately the issue was resolved after a full restart of autocad.  I'm chalking it up to a one-off bug.
