Does anyone know why some of the files have a preview window, when opening a file?

not quite sure why its there but it can be turned off by going to view just above the preview and then unticking preview.

If the file has a preview, it means that the screen-size was significant enough for a preview to be created when the file was closing. If you close down a file when it is minimized, I have found that the preview does not get saved. .dxf files do not seem to have preview available, only the .dwg files from what I have experienced.

so does the preview show you the drawing the way it was last saved?

It does. That's why it's a good idea to get into the habit of zooming to extents before saving and closing a drawing.

You can enable/disable the file preview in Options > Open and Save > Thumbnail Preview Settings.

Way to go you gurus, you done it again. Must have something to do with ACAD making a bmp image on the extents, also must be in the model frame too.
