Trouble with the tool palette

I was attempting to pick and place the imperial door from the tool palette and was unsucessful. I could pick and place other tools on the same palette without a problem. I am totally confused why this would happen. Any suggestions or advice.

Close out the drawing then close down AutoCAD. Reverse the process and try again. What happens?

does the command line say it's not found as if the source block has been deleted?
